What is it?
OrthoK is a non-surgical treatment to reshape the surface of the eye to reduce nearsightedness (myopia) and astigmatism. This is done by wearing customized retainer lenses to gently flatten the cornea during sleep. After the retainer lens is removed upon wakening, the cornea retains its flattened shape, and gives clear vision (typically 20/20 or better) without glasses or contact lens throughout the day. It works somewhat analogously to how braces correct deformed teeth. The patient needs to continue wearing these OrthoK lenses consistently to maintain the desirable corneal form for optimal vision. Most patients must wear them every night, but some patients (depending on their prescription and other eye surface characteristics) are fortunate enough to only need them roughly every other night.
Is this a new technology?
OrthoK has been around since the 1960’s. However, technological advances in lens materials, computer aided design, and computer aided manufacturing has improved the predictability and consistency of results. In 2002, the technology was granted FDA approval for treating nearsightedness and astigmatism for patients of any age.
Who is it for?
Dr. Koning believes that one of the best reasons for wearing OrthoK lenses is to reduce the progression of myopia in children. For instance, a 2015 meta-analysis (a statistical analysis that combines the results of multiple scientific studies) by Yuan Sun, et al, showed a 45% reduction in the axial length of eyes treated with OrthoK in kids between 6-16 years old, compared with age-matched norms. Unfortunately, there is a higher risk of some eye diseases later in life in people with high levels of nearsightedness, and reducing the progression of myopia in children can have life-long benefits both in terms of eye health and functionality. All that said, OrthoK can work well for adults of all ages, too!

Is Ortho-K safe?
It is FDA approved for all ages. All recent studies have shown safety and efficacy of this treatment. We understand that there has been some press about the risks of OrthoK. However, the risk of wearing retainer lenses at night is on par to the risk of other FDA approved contact lens overnight modalities. We have found complications to be exceedingly rare, and not serious. Of course, it is always important to follow our instructions regarding wearing, cleaning and follow-up schedule to ensure the successful treatment without complication.
How does Ortho-K compare with LASIK in correcting myopia?
Unlike LASIK procedure, Ortho-K is non-surgical and reversible. Both treatments reshaping the cornea to achieve clear unaided vision. Lasik is (relatively) permanent using laser surgery, while OrthoK is maintained by wearing customized retainer lenses. Children are not good candidates for LASIK because their eyes are still developing. People who have worn Ortho-K in their youth years can still choose Lasik surgery later. People who are not qualified for LASIK due to thin corneas may be good candidates for Ortho-K. In addition to being independent from glasses, the most important benefit for children is the potential of Ortho-K in myopia control in developing youth.
Am I a candidate?
FDA approved Ortho-k to correct up to -6.00 D of myopia and mild astigmatism. However, some recent new designs will allow correction for higher myopia and astigmatism. We will measure your eyes with a corneal topographer to assess the symmetry and steepness of the cornea, ruling out any corneal irregularity. During your annual exam, or at a free consultation, we will be able to discuss your candidacy with you.
How soon should I see well? How often do I need to wear Ortho-K lenses?
OrthoK lenses are designed for you to wear every night during sleep to achieve successful treatment. Most people notice improvement right after the first night. Depending on the pre-treatment prescription and corneal shape, each patient might need different length of time to reach fully stable vision throughout the waking hours, but this typically happens within the first 2-7 days. For patients with low myopia prescription, you might be able to skip one or two nights of lens wear after you have reached stability.
Will my insurance cover the treatment?
Unfortunately, Ortho-k treatment, similar to laser vision correction, is typically not covered by insurance. However, some insurances are exceptions to this rule, and you can use your HSA plans to cover the cost.
How much is the exam fee? Do I expect to pay this every year?
Ortho-k requires the doctors’ expertise, significantly longer chair time, training, and multiple office visits. Since these lenses are custom made, the initial cost is higher than regular contact lenses. The initial cost depends on the complexity of the case. Thankfully, once you are established as an OrthoK wearer, the cost per year actually averages slightly less than most soft contact lens wearers.
